"In an attempt to determine how often people die during medical procedures in Connecticut hospitals and doctors' offices, The Courant obtained, through Freedom of Information Act requests, more than 10 years worth of computerized death records from the Department of Public Health and the Office of the Chief Medical Examiner. More information was obtained about certain patient deaths by interviewing next-of-kin, doctors and lawyers and by searching court records."
