(SAN FRANCISCO CHRONICLE) This article provides an overview of the "major breakdowns in the nation's flu shot program" and provides a timeline that traces the significant events in this crisis.  WAITING FOR SHOTS -- "In a three-month investigation, The [San Francisco] Chronicle found that...[flu vaccine] delays resulted largely from sanctions levied earlier in the year by the U.S. Food and Drug Administration against two drug companies responsible for making almost half the nation's 75 million doses of flu vaccine.
